There are several types of crowns used in dentistry throughout the years.

First, what is a crown in dentistry? A crown, sometimes called a “cap”, refers to a full coverage restoration. In other words, it’s a cap or a covering for your tooth. The crown covers the remaining tooth structure, adding protection and stability to a compromised tooth. Your tooth might need a crown if your tooth has a large cavity, failed filling, or fracture. At our 10011 dental office, we usually do the crown procedure in 2 visits. The first visit, Keith Bracy, DDS will prepare your tooth to certain dimensions and digital scan will be taken. The scan is sent to a lab, where a crown will be designed digitally and made by a computer milling machine. This technology offers highly precise, beautiful dental crowns that fit well. Dr. Bracy finds that the digitally scanned crowns have less problems compared to those made with a traditional putty material. The turnaround time is also very quick! We get most crowns back from the lab within 3-5 days. During the lab fabrication time, you will wear a temporary crown fabricated by Dr. Bracy.

So what are your choices for crown materials? Most patients rarely ask what kind of crown, Dr. Bracy will fabricate, but it is important to know that you do have options in our West Village dental office.

Today, most patients have high esthetic demands and do not want to see any metal. Thus, metal-free crowns, often referred to as all porcelain or all ceramic crowns are increasingly popular. Two of the most popular “metal-free” crowns are called Emax crowns and Zirconia crowns. An Emax crown is made from lithium disilicate and is a very cosmetic looking restoration. This material bonds relatively well to tooth structure, so it might be a good solution for someone who has crowns that come off a lot. The Emax material is very strong, but not as strong as the all Zirconia option. Zirconia crowns, often referred to by the brand name “Bruxzir” are made from a crystalline zirconia oxide and offer relatively more strength. These crowns offer an excellent choice for a patient that grinds their teeth or has a history of breaking dental restorations. Zirconia crowns have become more esthetic and cosmetic looking, however, many claim the Emax crowns have higher translucency and esthetics. Bonding a Zirconia crown to your tooth is a very sensitive process, and the bond may be less predictable than an Emax crown if certain protocols are not followed.

A more traditional type of dental crown if referred to as a “PFM”. “PFM” stands for “porcelain fused to metal”. This type of crown has a proven track record in dentistry, with hundreds of thousands in service over several decades. This type of crown offers the strength and fit of metal (the inside of the crown) with the esthetics and beauty of porcelain (the outside of the crown). The one cosmetic problem with these crowns is that in certain situations, the metal can show through at the gum line. Have you ever noticed a person with some purple or metal colored discoloration at their gum line around a tooth? Chances are you are seeing the thin margin of metal that can show through from a PFM crown. For this reason alone, many patients and dentists have started to abandon this type of crown.

Want to show some dental bling or flare in Chelsea? If so, perhaps an all gold crown is the choice for you! Gold is one of the best dental materials, even in 2019! It is anti-bacterial and offers an excellent fitting crown with a good seal if done properly. Gold crowns can look very pretty, shiny, and nice in the eyes of the right beholder. Other patients find gold crowns to be rather ugly, but Dr. Bracy is a fan! The one downside of gold crowns is that they can be expensive with the rising price of gold, but it is still an excellent investment in your oral health.

root canal | endodontics | west village Yes, we preform root canal therapy in our West Village dental office. Root canal therapy, sometimes referred to as “endo”, are procedures within the field of Endodontics. During a root canal the nervous tissue of the tooth is cleaned out and the canals of the tooth are cleaned and shaped. A final filling material is placed within the canals to seal them and keep bacteria out. Keith Bracy, DDS performs many types of root canal procedures. However, some teeth may require the skills of a root canal specialist, called an “endodontist”. The teeth that require a root canal specialist are many molar teeth and those with extra canals or unique canal systems. Don’t worry if you need an endodontist! We can coordinate one to come to our West Village dental office or can refer you to one in the neighborhood if needed.

It is important to return general dental office after a root canal within 2 weeks to have a final restoration placed on the tooth. The root canal specialist will leave the tooth with a temporary filling. A final, stronger restoration must be placed on the tooth to keep it protected. In most cases, this is a crown. Yes, we are excited to introduce our dental hygienist, Shirley Tam to our West Village, NYC dental practice. Shirley has years of experience in dental hygiene and is eager to meet all of our new and existing patients. Dental hygienists have at least 2 years of academic training the field of dental hygiene and specialize in dental cleanings, non-surgical periodontal treatments like “deep cleanings” (also known as quadrant scaling and root planning), oral hygiene instruction, and giving you overall dental advice such as how to lower your risk for dental cavities. Shirley also helps out with our in-office Kor whitening system! Shirley is currently available for Friday appointments. The other days of the week, Dr. Bracy is happy to perform your dental cleanings and take care of your dental hygiene needs.

Our philosophy on dental cleaning visits is patient-centered care. We believe dental hygiene is important and visits should not be rushed! We book at least 1 hour of time for any dental cleaning and want you to develop a trusted relationship with your hygienist. If your care requires a specialist such as an endodontist (root canal specialist), we can often have the specialist come to our office instead of being referred out.

Invisalign Clear Braces

No, it is not advised to drink coffee with your Invisalign clear aligners in place! The color of the coffee will stain the clear aligners, and the warmth can cause distortion in the aligner material. If the Invisalign aligners are stained, they will be unsightly. If the Invisalign aligners are distorted they will not work correctly.  Stop by our West Village dental office to ask Dr. Bracy about more tips and tricks on Invisalign treatment!

In most cases, the answer is yes. A dental implant cannot be moved like a regular tooth as the implant body is fixed, integrated into your jaw bone. Invisalign, clear aligners, and traditional orthodontics cannot move dental implants. Thus, if you have a dental implant and are considering orthodontics, your treatment plan will be made taking this into consideration. If you have a one or two dental implants, in most cases the movement of teeth adjacent to the implant can be planned for a successful outcome. However, if you have no remaining natural teeth or a high number of dental implants in certain positions, Invisalign or clear aligner therapy is probably not for you. The only way to know is to have a consultation with a dentist or orthodontist and have the details of your particular situation examined. Keith Bracy, DDS does not recommend it.  Contrary to the advice of dentists and orthodontists, using an Invisalign aligner to hold your whitening gel may lead to poor results or be unsafe. Why? Because a clear aligner does not provide the proper seal for whitening gel to stay in the tray. Look closely at the seal of an Invisalign aligner – the aligner usually ends before the gum line and there often isn’t a tight seal between the Invisalign aligner and the tooth due to the rigidity of the Invisalign aligner material. Thus, whitening gel can easily escape the Invisalign aligner and saliva can also enter. Both of these scenarios lead to inconsistent and poor whitening results. Obviously, if gel can escape the Invisalign aligner, it is not able to do its job and can cause burning on the gums or a bitter taste in your mouth. Furthermore, if saliva can enter the Invisalign aligner, it will wash out the bleaching gel and can also degrade the active ingredient very quickly.

If you want to whiten your teeth and you are considering Invisalign, do it the right way! Try KoR Whitening at Keith Bracy, DDS. You will immediately see the difference between a Kor seal tray and an Invisalign aligner. One of the reasons why Kor is considered the most effective whitening system on the market, is because the whitening tray is so well made. Whitening gel cannot escape the Kor seal tray and saliva cannot enter it, thus the tooth is in contact with whitening gel all night, leading to outstanding results. Conversely, an Invisalign aligner was made to straighten teeth, not hold whitening gel against the tooth.

In the ideal jaw and tooth position, your upper teeth will extend slightly over your bottom teeth. Also, in the ideal facial profile, your lower jaw does not extend out farther than the rest of your face. However, New York patients are not perfect and some of us live most of our lives with a non-ideal tooth or jaw position, known as an underbite. Should I correct my underbite?

The most obvious reason to correct an underbite is esthetic reasons. Improving or reducing the appearance of an underbite can improve your confidence! However, correcting your underbite isn’t just so you will look like the most beautiful person walking down West 12th Street! It is also for health reasons. Underbites and non-ideal bites can cause excessive wear of teeth and lead to dental problems down the road such as excessively worn or fractured teeth. Invisalign | greenwich village nyDental coverage for Invisalign and orthodontic treatment varies widely. The only real way to know if your plan covers Invisalign is to do a benefits check. Dr. Bracy’s staff is happy to contact your insurance carrier and let you know the details of your specific plan. In most cases, all we need is your insurance carrier name, member ID and date of birth.

Some PPO dental plans have coverage rates of $1,000 - $3,000 for Invisalign or orthodontic treatment. Often this benefit is limited to specific age ranges such as below age 19 only. But in other cases, all ages can receive the benefit. It depends on the contract setup between your employer and insurance company.

If you don’t have dental insurance or if your dental insurance does not provide any coverage for Invisalign, do not worry! There are many payment options for this type of dental treatment. Invisalign treatment time largely depends on patient compliance with wearing the aligners, complexity of tooth movement, and age of the patient. Highly motivated patients that wear the aligners 22+ hours a day have the best results with Invisalign and a smaller chance of running into treatment challenges. Also, some types of tooth movement are more complex than others and may take longer. Younger patients may also experience faster tooth movement compared to older patients. However, do not let your age discourage you from Invisalign or orthodontic treatment – it is never too late to get the smile you’ve wanted.

One of the major advantages of Invisalign versus other clear aligner brands is its track record with weekly aligner changes. For many years, dentists and orthodontists recommended changing Invisalign aligners every two weeks. However, in 2016 Invisalign released study data supporting one week aligner changes. If you are worried about the length of time for your clear aligner therapy, do not stress until you have seen your Invisalign treatment plan. If it is, then Dr. Bracy will take the necessary x-rays, photographs and scan of your teeth (orthodontic records). This information is sent to Invisalign for treatment planning. After your Invisalign treatment plan is developed, you will meet with Dr. Bracy again to see the results, learn about the estimated treatment time and discuss your payment options.

Invisalign | clear braces | west village At Keith Bracy, DDS the rate of Invisalign aligner change is usually 7-14 days. Each Invisalign clear aligner must be worn for the majority of the day (20 + hours, ideally 22-23 hours per day) and most of our patients are able to move on to the next Invisalign clear aligner in 7-14 days. The rate of Invisalign clear aligner change is largely dependent on the patient’s age, the complexity of tooth movements, and patient compliance. A younger, compliant patient with simple tooth movement will be able to change their Invisalign clear aligners more frequently than an older, less compliant patient with complex tooth movements. Align Technology, the manufacturer of Invisalign, has presented much data supporting 7 day clear aligner changes.

The major advantage of the Invisalign system, is that it is in person-doctor directed and the treatment plan is personalized to your unique orthodontic needs. The rate of Invisalign clear aligner changes can be customized to your unique needs.

Compliance with wearing the Invisalign aligners is one of the most important factors in the success of your treatment. Invisalign is nearly invisible, very comfortable, and it will work, but the patient must be motivated to wear the aligners!

No, you should always save your at the minimum your last Invisalign clear aligner. One of the advantages of the Invisalign branded clear aligners is that they are doctor prescribed and you have your progress monitored by your dentist or orthodontist. You should bring the last Invisalign clear aligner with you to your progress appointments. The old Invisalign aligner can be very helpful to the dentist in case anything goes off course during your treatment. Sometimes a new Invisalign aligner isn’t fitting well, and your dentist can determine if you should go back to the old Invisalign aligner or if it is safe for you to move on to the next one.

At Keith Bracy, DDS we do progress tracking appointments for Invisalign about every 4-6 weeks, depending on the complexity of the case. Dr. Bracy will evaluate your Invisalign progress at these visits. Dr. Bracy will check to see if all your attachments are intact, if your aligners are fitting well, if your teeth are moving as they should be, and provide you motivation to keep on track with your treatment. Dr. Bracy will also make sure your treatment is progressing safely – the health of your teeth are of utmost importance during any orthodontic treatment.

How often should you change your Invisalign clear aligner? Dr. Bracy will provide a customized Invisalign aligner schedule for you. The rate of Invisalign aligner change is personalized and largely dependent on your age, complexity of the tooth movement, and patient compliance.

Patient Motivation – The success of your Invisalign depends on your compliance! You must wear the aligners 22-23 hours a day to see the best results. If you find it difficult to start a new habit or only plan to wear the aligners half the day, the treatment will not work. 

Frequent Brushing – You need to take the Invisalign aligners out to eat or drink. Before you place them back in, you need to make sure your teeth are clean. Some patients find it annoying to brush before placing them back. However, many see the brighter side of this “con” – you’ll be brushing your teeth frequently and also avoiding snacks!

Vivera is the brand name of clear retainers manufactured by the makers of Invisalign, Align Technology. They are the premium choice for long-lasting clear retainers. Unlike fixed, wire retainers they can be removed for easy cleaning. After the completion of your Invisalign treatment, Dr. Bracy will fabricate the Vivera retainers for you in his West Village, NYC dental office.  They look and appear similar to your last Invisalign clear aligner, but they are made from a much sturdier, clear material. Dr. Bracy will discuss with you the recommended wear time for your new Vivera retainers. Often you must wear them full- time after completing your Invisalign treatment, then gradually taper to just overnight wear.

Vivera retainers are not just for those who recently completed Invisalign! If you have lost your retainers or are looking for a replacement option for your current retainer, ask Dr. Bracy about Vivera retainers. 

It happens to the best of patients! Lost, misplaced, or damaged clear aligners are common throughout your Invisalign treatment plan. In our West Village dental office, we’ve heard it all from patients. It might sound comical, but often pets and dogs are attracted to the used aligners due to the scent of food and their owners. A dog can do real damage to your Invisalign clear aligner!

If you are a current patient of Keith Bracy, DDS and you had your Invisalign clear aligner lost or damaged, don’t worry! Dr. Bracy provides replacement aligners free of charge to the patient throughout your Invisalign treatment plan. Please call the office and ask Dr. Bracy if you should wear your last clear aligner while you wait for your replacement or if you should not wear any aligners at all. Often Invisalign can replace the lost or damaged clear aligner in a few business days. We understand that it can be stressful to lose an Invisalign aligner since suggest wear time is 22-23 hours a day!

At the end of your Invisalign treatment in our West Village dental office, you will start on your Vivera retainers. You will receive 4 sets of the Vivera retainers and given a personalized treatment plan for wear time. At this time, we do not provide free replacement for lost or damaged retainers – just the clear aligners during Invisalign treatment.

We would encourage you to not let price be the ultimate deciding factor in your oral care needs and to do your research. Often, the cost of the retainers or additional aligners are not included and can be a surprise bill at the end of your Invisalign treatment. 

If you are asking yourself this question, chances are you are looking to start a journey with clear aligner therapy! Before you start Invisalign or any clear aligner therapy, you should have a general dental check-up and clearance for orthodontic treatment? Why? Because doing Invisalign or clear aligner therapy over unhealthy teeth has serious risks! In fact, wearing clear aligners over teeth with untreated gum disease can lead to loss of the tooth or make it very loose. Also, teeth with cavities should be treated before starting clear aligner therapy. A dentist should also do a physical examination of your jaws and let you know if there are any other contraindications to orthodontic treatment. Keith Bracy, DDS does not recommend it.  Contrary to the advice of dentists and orthodontists, using an Invisalign aligner to hold your whitening gel may lead to poor results or be unsafe. Why? Because a clear aligner does not provide the proper seal for whitening gel to stay in the tray. Look closely at the seal of an Invisalign aligner – the aligner usually ends before the gum line and there often isn’t a tight seal between the Invisalign aligner and the tooth due to the rigidity of the Invisalign aligner material. Thus, whitening gel can easily escape the Invisalign aligner and saliva can also enter. Both of these scenarios lead to inconsistent and poor whitening results. Obviously, if gel can escape the Invisalign aligner, it is not able to do its job and can cause burning on the gums or a bitter taste in your mouth. Furthermore, if saliva can enter the Invisalign aligner, it will wash out the bleaching gel and can also degrade the active ingredient very quickly.

This type of secured denture is called an overdenture.


  • After your exam and treatment phase is over, the next step is an impression or digital scan of your mouth. The initial impression is basically a mold of your jaws. Many times complete dentures will take 2 visits for impressions. This first impression is used to fabricate a very precise tray, used to take the second impression.   •  The third step is a try in of the wax denture bases. At this visit, Dr. Bracy will see how the denture bases fit your mouth and will gather data on how our jaws come together and various measurements of where the teeth should be placed. Denture Hints and Tips

  • Always keep your denture clean and hygienic. Wash them with a toothbrush and warm water and mild soap. Use denture cleaning tablets when you soak it. You should clean it before and after use and always rinse the cleaning agent well.
  • Do not let your denture dry out. When you take it out of your mouth, soak it in water or denture cleaning solution. Letting it dry out can make the acrylic more brittle and uncomfortable to wear.
  • Do not sleep in your denture. Your gums and mouth need time to rest too! Wearing your denture at night is unhygienic and can allow for fungal infections to grow beneath the denture.
  • Give it time to adjust to! No one was born with dentures. They take several weeks to adjust to before you can eat, drink and speak with them like second nature.

There are also brands for the flexible material – Valplast is a common brand. The material of the flexible denture basic is translucent and custom made to the shade of your gums. The denture does not have metal clasps but instead has gum colored flexible clasps and rests on your gums for support. Traditional partial dentures, unlike flexible partial dentures, are made from metal and acrylic bases. The metal can be unsightly and many patients worry if the metal clasps can be seen when they smile. The acrylic of a traditional denture can match your gum shade well, but it isn’t translucent and life-like like flexible dentures. Flexible dentures are also a good option for those with acrylic or metal allergies.

At Keith Bracy, DDS we have seen dentures in service for up to 20 years at the extreme. However, there are many factors that affect the life span of your denture. Traditional dentures are made from acrylic and resin (plastic) teeth. These are very durable materials, yet they aren’t immune to aging.

 Much of your denture life depends on how well you take care of it. Dentures need to be cleaned with warm water and soap at least twice a day or before and after inserting. Alternatively, many over counter products like denture tablets can be used. Cleaning your denture allows for removal of plaque, bacteria and stains – all which can shorten the life of your denture. Bring your denture to Dr. Bracy at regular dental check-up appointments. Just because you are missing your teeth and have a denture, does not mean you can skip dental appointments! An exam of the fit and function of your denture,  as well as oral cancer and pathology screenings,  must still be performed. Dr. Bracy and his hygienist can also perform a professional cleaning of your denture and review use and care instructions.

Another factor that determines how long your denture lasts is changes in your jaw bone and gums. Over time, some patients experience remodeling of the bone in their jaws. If the bone changes or shrinks away, then the denture will lose its fit. You might notice this if your denture starts to be unstable, lost suction, or moves around more. In some cases, the existing denture can be refitted in a procedure called a denture reline. In other cases, it might be better to make a new denture, if the changes are significant enough. Another option to consider in the case of shrinking bone or poorly fitting dentures, is dental implants to secure the denture. Sometimes just 2 dental implants can make a huge difference in securing a lower denture, and 4 dental implants in an upper denture.   4. Insertion Day – High end, quality dental laboratories usually have a speedy turnaround time for porcelain veneers. Dr. Bracy will only use well-regarded dental laboratories for your case. In most cases, Dr. Bracy will send you to the dental laboratory so you can meet the technicians and ceramists doing the fabrication.
